Ensuring a natural hairline in hair transplantation is directly related to 3 factors:
- Density and volume of the transplanted hair
- The direction and angle of transplantation
- Planning of the front hairline.
The hair transplantation team shall carefully work during the operation to establish all these 3 factors and each graft shall be transplanted to the most appropriate place at the most accurate angle.

What is the front hairline?
The front hairline (foreheadline) is the area where the forehead connects with the hair. The place of the front hairline and thus, the largeness of the forehead change depending on the facial features. While the front hairline differs from one person to another, the hairline has certain common features. The front hairline, which may vary depending on age, gender, and facial features, can cause a person to look younger or older than he/she actually is. The front hairline may be quite receding even in persons who are not experiencing hair loss and the largeness of the forehead may be disturbing for the individual. At this point, the “golden proportion” comes into play to determine the present and the expected states.
Two criteria shall be taken into consideration when planning the front headline (forehead line):
- Age Factor,
- The Golden Proportion.
Researchers have shown that people focus on the hair and forehead area just above the skin’s wrinkle level. When the front headline is heavily receding, the person may look older than he/she really is. When the front headline is too far frontward, this can disturb the natural look and cause the aesthetic features of the face to disappear. Taking the age factor into consideration will help with the elimination of these problems.
The second criterion is the golden proportion which has a key role in all aesthetic operations today. In golden proportion, the best front headline is determined in terms of aesthetics.
How are Calculations Performed?
The front headline, which is a key factor defining our look, is determined during the planning of the hair transplantation process. Calculations and determinations should necessarily be performed by a specialist physician.
The points to consider in the calculation are as follows:
- Assessment of the scarcity of hair based on the volume of the donor area
- Requests of the person requiring hair transplantation
- Whether the hair loss process has been completed or is still ongoing
- Age of the patient
- Head and facial features of the patient
In patients with intense hair loss or in elderly patients, keeping the hairline a little higher will support a natural look.
How Should Hairs Be Lined in the Front Hair Area?
As this area is at the front and at eye level, it is more important than the other transplantation areas. The angle and direction of the grafts transplanted in the front hair area have key roles in the hair transplantation operation. Hair strands cannot be lined in a single row in this area. To prevent any situation that will disturb aesthetics, hair strands are lined in a mixed and natural way. The asymmetrical lining of the front hairs will bring about a more natural look.
In patients with hair loss only on the front side of the head need to have denser hair follicles in the 2-cm-large area just behind the front headline. There should be 2 or 3 hair strands in this area. In the front headline area, grafts with a single hair follicle should be preferred.
Double hair follicles and then, triple hair follicles should come following the single follicle. During transplantation, density should be increased by going backward from the headline.
